Diabetes Prevention Program Information Session

Join MHCS diabetic educators for this free information session and learn more about the Diabetes Prevention Program.  Sessions offered Thursday, February 6, 2020 at 9 a.m., Noon  or 3 p.m. Held in the Seward Family Medical Center lower level Vahle conference room.

MHCS Diabetes Prevention New Session begins March 3, 2020

Medical Eligibility Criteria:

  • 18 years of age or older
  • Body Mass index > 24


One of the following prediabetes indicators listed below:

  • Fasting plasma glucose 100-125 mg/dl

  • Oral glucose tolerance test (75gm.) with 2 hour plasma glucose 140-199 mg/dl

  • A1c of 5.7 -6.4

Clinically diagnosed gestational diabetes mellitus during a previous pregnancy 

Screening Test - The Center for Disease Control (CDC) Prediabetes Screening Test may also be used to qualify a person for the diabetes prevention program

Yearlong lifestyle change program:

  • 16 weekly meetings, followed by 8 monthly meetings
  • Meetings are one hour in length
  • Cost to attend the program is $150.00

Program goals:

  • 7% weight loss
  • 150 minutes of exercise weekly
