Falls Risk Assessment

Falls are not a normal part of aging, they can be prevented. If you are an older adult, there are things you can do to keep yourself from falling and stay independent longer.

  1. Talk to your health care provider and schedule your Medicare wellness visit.
  • Tell your medical provider if you have fallen, feel unsteady when standing or walking, or afraid you might fall.
  • Ask your medical provider to review your medications to see if any might increase your risk of falling.
  • Ask your medical provider about health conditions that can increase your risk for falling.
  • Ask your medical provider to check your eyes at least once a year and update lenses as needed.

2.  Stay Active.

  • Do exercises that make your legs stronger and improve your balance.

3.  Make your home safer.

  • Get rid of trip hazards and keep floors clutter free.
  • Add grab bars in the bathroom.
  • Have handrails and lights installed on all staircases.
